Any Proposer having questions regarding this RFP shall submit those <br />questions in writing to Mr. Ray Thoma.s, the Director of Public Service for the <br />City of North Olmsted. <br />B. Answers will be provided to a.ll Proposers as a written addendum. Proposers <br />are not to rely an oral discussions, instructions or answers. .Any oral <br />discussians, instructions or answers will not be binding on the City. <br />IV. PROPOSAL DEADLINE <br />Six (6) bound copies of a proposal must be received by the City of North <br />Olmsted, 5200 Dover Center Road, North Olmsted, 4hio 44070, no later than <br />12:00 noon, June 22, 2042. All proposa.ls must be addressed to the attention of Mr. <br />Ray Thomas, Director of Public Service. AIl proposals shall be plainly marked on <br />the outside "DESIGN PROFESSIONAL SERVICES PROPOSAL". <br />V. DESIGN PROFESSI4NAL SCOPE OF SERVICES <br />The successful Proposer will provide design professional scope of services as <br />outlined in APPENDIX B to this RFP. The services will be provided under the <br />Agreement between Owner and successful Proposer. <br />VI. CONTENTS OF PRUPOSALS <br />Statements of qualifications should include informa.tion regarding: <br />A. Firms history; <br />B. Experience in providing design professional services for projects similar to <br />and larger than the Pump Stations Influent Control Gates Project; <br />C. Education and experience of staff who will be involwed in the Project; <br />D. Study, evaluation, preliminary design and detailed design phase services <br />offered by your firm; <br />E. Firm's general liability insurance and financial responsibility; <br />F. List any pending or current legal proceedings such as mediation, arbitration <br />or suits; <br />G. References; <br />H. Other services offered by the firm; and <br />1. Documentation indicating firm's compliance or non-compliance with each of <br />the standards listed in Appendix E.
