ARTICLE 12 <br />HEALTH INSURANCE <br />12.1 The employer shall prcvide either individual or family medical insurance <br />coverage at the employee's option for each fuli-time employee. The employer shall have the <br />option of seeking alternate health insurance providers or plans and will attempt to offer at least <br />two different plans. Basic hospitalization will be consistent with and similaz to those plans <br />offered on July l, 1994. Basic hospitalization will be that offered to other employees of the City <br />of North Olmsted. The employer has the right to negotiate copayment and prescription drug rates <br />with providers to lower the cost to the employer as well as the employee contribution. <br />12.2 Januazy lst, 2001 to July ls`, 2001. A full-time employee participating within either <br />an individual or a family plan shall contribute an amount equal to 12% of the plan premium or <br />the expected cost of such medical plan and the Employer shall contribute 88% of such cost. <br />From July lst, 2001 through the duration of the agreement, the employee shall pay 13% and the <br />Employer shall pay 87% of such cost. <br />All employee contributions, as required by this Article, shall be withheld via payroll <br />deduction not later than the first pay period each month for a11 the employee percentage <br />contributions sta.ted above. <br />12.3 In conjunction with participating within a medical plan, the employee may <br />subscribe to and participate in the following optional health benefit coverage(s). <br />1. Dental <br />2. Vision - Optional Plan 41 (Vision plan which was in effect August 1, 1994) <br />Optional Plan 42 ( Vision plan similar to that offered throughout Union <br />Eye Care). <br />3.
