14.6 The Employer shati require an Employee to furnish a standard form identifying the reason for the <br />use of sick leave as per 14.5 and signed by the Employee. <br />14.7 If inedical attention is required, the Employee shall be required to furnish a statetnent from a <br />licensed physician or psychologist notifying ttte Employer that the Employee was unable to <br />perform his duties. An_y firefighter who is sick or disabled for more than two (2) consecative tours <br />of duty or forty (40) hour Employee who is sick or disabled for five (5) consecutive worldng days <br />may be required to secure and submit a physician's release certifying thai they are fit to remrn to <br />work. This retease must be submitted to the Employee's Officer in Charge before the Employee <br />wiil be pernutted to return to work. <br />14.8 Emgloyees shall be permitted to contribute sick leave hours to a sick leave bank for use by any <br />Employee who has exhausted all of his own sick leave under the foilowing conditions: <br />A. The Sick Leave banlc shail have a inaxiinum balance of fifteeit hzuzdred (1500) hours. <br />B. Employees that have suffered any illness or injury in the scope of their emplovment and have <br />exhausted the benefits of Articie XVI or were not covezed by the prrn isions of Articie XVI <br />mav d.raw from the sick leave bank after their own sick leave bank has been exhausted. <br />C. Employees that have suffered any iltness or injury to themselves or their immediate family <br />outside of the scope of their employment may draw from the sick Ieave bank ouly after the <br />Employee has exhausted all of their own sick leave bank vacation and hoiidays. <br />D. Any Employee that contributes sick le.ave hours to the sick leave bank shall not lose anv sick <br />leave bonus as grar?ted by section 142. <br />E. No Employee shail be pernutted to contribute more then one hundred (100) hours during his <br />term of employment. <br />14.9 The Emplo_Yer may require any Employee requesting paid sick leave to furnish substantiating <br />evidence or a statement from his/her attending physician certifying that absence from work was <br />required due to one of the reasons set forth in 14.5. Such certification must be preseiuted whenever <br />sick leave is requested for more thau rivo (2) consecutive tours of ctuty for Employees who work <br />an average of 51.7 hoius per week and for five (5) consecutive worldng days for Employees on a <br />forty (40) hour work week <br />4: <br />21 <br />
