E«iployer shall contribute eighty-seven (87%) percent of such costs. The Employee <br />contribution shall be withheld via payroll deduction not later than the first pay period each <br />month. <br />19.3 In conjunction with participation within a medical plan, ttte futl-time Emplayee may subscribe to <br />and participate in the follo«zng optional health benefit coverage(s): <br />Dental <br />Vision - Option Plan #1 (vision plan which was in effect August I, 1994) <br />Vision - Option Plan #2 (vision plan similar to that offered through Union Eye Care) <br />Dental and Vision (Option Plan #1 or #2) <br />Full-[ime Employees participating and subscribing to the optional health benefit listed above <br />witlun this Section shall contribate an equal amount to that which is listed in Section 19.2(A), (B) <br />and (C) of this Article. Employees shatl be required to participate within the optional benefit <br />coverage listed widiin tlus Section. The Employee contribution shall be withheld as a payroll <br />deduction not later than the first pay period of each month. <br />19.4 Expected cost of premium is defined as the cost of all medical, hospital, prescription drug and <br />related fees including, but not 1'uiuted to, administration fees. <br />19.5 The Empioyer reserves the right to establish plans, enrollment periods and regulations. <br />14.6 The Employer shall estabiish a committee of one (1) representative from each of the Cirv's <br />bargau,ing units, if they choose to be represented and a mnumum of one (1) representative of the <br />Employer. T'he bargaining unit's role as a participant in the aforementioned commiitee shall be <br />advisory oiilv. <br />19.7 Ali changes affecting the plan coverage's co-payment, deductibles, providers' enroilment periods, <br />and regulations will be shared with the bargaining unit prior to maldxig such changes. <br />19.8 It is further agreed that if any bargaining unit employed iry the City of North Olmsted, in <br />negotiations held subsequent to this Agreement and for the same time period covered under this <br />Agreement, negotiates to conclusion a lower percentage of Employee contribution to the health <br />Insurance Plan, that lower percentage shall apply to the unit covered under this Ageement Said <br />28 <br />
