CONTENTS 4F PROPOSALS <br />A. Statements of qualifications should include information regarding: <br />1. Firms history; <br />2. Experience in providing information systems consulting services for projects <br />similar to and larger than the City's project including identification and description <br />of projects within the previous five years, timeliness of performance and outcome; <br />3. Education and experience of all staffwho will be involved in the Project; <br />4. List of services offered by your firm; <br />5. Firm's general and professional liability insurance and financial responsibility; <br />6. List any pending or current legal proceedings such as mediation, arbitration or <br />suits; <br />7. References especially in government related systems. <br />B. Cost of Services including, but not limited to, hours and cost by task listed in <br />Section VI.
