Patel, P.E. <br />Polytech Inc. <br />1744 Payne Avenue <br />Cleveland, Ohio 44114 <br />Re: Request for Proposal <br />Pavement and Sewer Improvements <br />Virginia Avenue and Summerland Avenue <br />Dear Mr. Patel: <br />Ed Boyle, Mayor <br />Phone: (216) 777-8000 <br />I am requesting a Proposal to provide professional engineering services for the design of <br />the upgrade of the infrastructure of Virginia Avenue and Summerland Avenue, by repair <br />or replacement of deteriorated sanitary and storm sewer laterals, construction of a new <br />storm sewer main on each street, removal of existing asphalt roadway, preparing the <br />? subgrade, pouring a new reinforced concrete pavement with curbing, new concrete <br />driveway aprons, handicap curb ramps and sidewalks, and restoration of the tree lawns, <br />which will eliminate the current shallow ditches across the front of each property which <br />? collect storm water and carry it to a lazge ditch which runs north and south along the <br />border with the City of Fairview Pazk. The basis of design for the storm sewers shall be a <br />25-year storm. <br />? The Proposal shall include the following items in the Scope of Services: <br />1. General CondiHons: <br />a. Work shall not be subcontracted without prior consent of the City in writing. <br />? Since the project is partially funded by a Community Development Flood <br />Grant, the City will furnish certain forms pertaining grant requirements <br />for the bid specifications. The Consultant will also be required to comply <br />? with County requirements of the Community Development Block Grant <br />programs, such as Affirmative Action and Non-Discrimination Regulations. <br />? Certification with the County as a Minority Business Enterprise is required. <br />b. Additional billable services shall not be performed without prior consent of <br />the City in writing. <br /> <br />.-qw,
