required to submit proof acceptable to the City of his ability to perform the work of his job upon <br />the termination of the sick leave. Any dispute as to the ability of an employee to perform the <br />work of the job shall be determined pursuant to the medical examination procedure in the <br />General Leave Provisions, Section 5. <br />Section 3. Paid Sick Leave: An employee who has completed his probationary period <br />shall be granted sick leave for personal illness or injury as fotlows: <br />(a) Paid sick leave shall be earned and accumulated at the rate of <br />one and one fourth (1 1/4) days for each month; <br />(b) Pay for sick leave shall be at the employee's straight time <br />hourly rate (or portion thereof if absent for less than a full day); <br />(c) If an employee is absent on sick leave for more than three (3) <br />consecutive work days, the City shall require the employee to <br />present medical evidence that the employee was under a <br />doctor's care and is able to return to work. The City will review <br />the attendance records of employees periodically, and at least <br />once in each three (3) month period. If it believes that an <br />employee is developing an attendance problem, it will notify the <br />employee in writing that medical evidence will be required for <br />absences of three (3) days or less. When an employee's record <br />becomes satisfactory, the requirement for such medical evidence <br />for short term absences will be removed, and the employee will <br />be notified in writing. <br />(d) Employees may use up to fifteen (15) work days per year of <br />earned and accumulated sick leave if necessitated by illness <br />in the employee's immediate family. For purposes of this subsection, <br />"immediate family" shall be interpreted to be spouse; children; <br />parents of the employee, regardless of the place of residence; <br />and other relatives living within the household of the employee. <br />(e) When an employee with five (5) or more years of service retires, <br />having received P.E.R.S. pension, dies or takes disability retirement <br />and has accumulated sick leave time, the retiring employee or his <br />estate may convert one-half (1/2) of the accumulated sick leave <br />time to pay on the basis of his rate of pay at the time or retirement, <br />but the maximum amount paid shall not exceed the following: <br />V*W <br />15 <br />....,.._ <br />10 .4.. ? ?,,?. . .,
