Employee's on an unpaid-sick Ieave, parental leave, or has exhaust.ed his accumulated sick leave shall not be entitled to any benefits, except hospitaZ/surgical benefits which shall continue to be provided by the E-mployer fox a period af t?me »at to exceed ninety (90) days. Duxing the afbrementioned ninetX (90) day time period, the emploXee shall be required ta pay his pxaportioanate shaxe af the cast as required within Article 28, of the Agreement. The pxdportianate cost shall be paid tv the Emplayex by the employee in acivance an or before the first day of the month. The EmployeX shall cantinue to pay its proportXonate shax-e of the Gost as required within Articie 28, of the Agreement during this ninety (90) day period. AEter ninety (90) days, the employee shall be required to pay to the Emp].cyer, the fuxl cost of the cost of such hospital/surgical benefits in advance an a monthly basis, oommencing an the ninety-first (51st) day. Any employee that fails to pay his proportzonate share or the full cost as described shall be dropped as a participant from the group health plan for nQnPayment, as x-equired by thxs Agreement. An employee that is dropped from the group health plan far non-payment shall ozaly be eligzbZe to participate after the conc3.usion of tr.e leave and anly upon his active return to wark. Employees shall be subject to al2 af the tex-ms and conditions of the group health plan. ARTICLE 22: INJURY LEAVE SECTION 1: In the event that afull-time employee or the North Olznsted Pol_ice Department should be injUxed while in the line of duty due to a sudden and accidental occurrence, and such injuxy has so izacapacitated said membex of the Folice Department that he is temporarily unable to woxk, then the Director of Public Safety, after determinxng that said injury is wdrk-rel.ated and is of a temporary nature, znay authorize ninety (90) days sick leave. If after such ninpty (90) days si.ck Xeave period said emp].ayee is still temporarily incapacitated, then the Director of Public Safety shall recammend to Gouncil whether or not to continue saxd salary. Council shall forthwith review the matter and, by majority vote, determine whether the employee shall contiriue to receive his full salary durinq recupexation. For the purpose of this paz-agraph, a sudden and accidental injury shall include such incidents as an automobile accident, assauit, gun shat wound and the liKe. It may not be considerea as sudden and accidental zf an injury or incapacitation results from lifting, s2ipping, trippzng ox- falling.
