participating within either an individual or family plan shall contribute an amount equal to five <br />--- percent (5%) of the plan premium or the expected cost of such medical plan and the employer <br />shall contribute ninety-five percent (95%) of such cost. The employee contribution shall be <br />withheld via payroll deduction not later than the first pay period each month. <br />Effective August 1, 1995 and through the duration of this Agreement, full-time <br />employee(s) participating within either and individual or family plan shall contribute an <br />amount equal to eight percent (8%) of the plan premium or the expected cost of such <br />medical plan and the employer shall contribute ninety-two percent (92%) of such cost. <br />The employee contribution shall be withheld via payroll deduction not later than the first <br />pay period each month. <br />Section 243. In conjunction with participating within a medical plan, the employee may <br />subscribe to and participate in the following optional health benefit coverage(s): <br />l. Dental <br />2. Vision - Optional Plan #1 (Vision plan which was in effect August 1, 1994) <br />Optional Plan #2 (Vision plan similiar to that offered through Union Eye Care) <br />3. Dentat and Vision (Optional Plan #1 or #2) <br />Employees participating and subscribing to the optional health benefits listed above within this <br />section, shall contribute an amount equal to that which is listed within Section 24.2. of this <br />Article. Employees must participate in the medical plan to be permitted to participate within the <br />optional benefit coverage listed within this section. The employee contribution shall <br />be withheld as a payroll deduction not later than the first pay period of each month. <br />Section 24.04. Expected cost or premium is defined as the cost of all medical, hospital, <br />prescription drug, and related fees including, but not limited to administrative fees. <br />Section 24.05. The Employer reserves the right to estabiish all plans, enrollment periods <br />and regulations. <br />Section 24.06. The Employer shall make an earnest effort to establish a committee <br />consisting of one (1) representative from each of the city bargaining units, if they chose to <br />be represented and a minimum of one (1) representative of the employer. The bazgaining <br />unit's role as a participant in the aforementioned committee shall be advisory only. <br />Section 24.07. All changes affecting plan coverage's, co-payments, deductibles, providers, <br />enrollment periods and regulations will be shared with the bargaining unit prior to making <br />such changes. <br />It is further agreed that if any bargaining unit employed by the City of North <br />Olmsted, in negotiations held subsequent to this Agreement and for the same time period <br />covered under this Agreement, negotiates to conclusion a lower percentage of employee <br />contribution to the Health Insurance Plan, that lower percentage shall apply to the unit <br />covered under this Agreement.
