<br />(d) Emergency/non-routine medical, dental surgery or optical <br />surgery of the employee; and <br />(e) Sickness or emergency medical treatment of the <br />employee's immediate family. The term "immediate family" <br />shall be interpreted as meaning parents, parents of spouse, <br />spouse, and children of the employee, regardless of <br />place of~residence, as well as other relati~aes living <br />in the employee's home. The maximum sick leave which may be <br />granted under this paragraph for family illness may be no <br />more than eighty (80) hours per year for employees who <br />have been with the City more than twelve (12) months. <br />Department heads may require a medical certificate before <br />paying any employee under this paragraph. Falsification <br />of either a written, signed statement or a medical <br />certificate shall be grounds for disciplinary action <br />including dismissal. <br />(f) Sick leave may be used to attend the funeral of a half-brother, <br />half-sister, brother-in-law or sister-in-law, for a maximum <br />of three (3) days. <br />Section 18.15. Any abuse of sick leave or the patterned use of sick <br />leave shall be just and sufficient cause for discipline, including <br />,• discharge, as may be determined by the Employer. {Example: Mondays, Fridays, <br />day after payday, etc.) <br />~ Section 18.16. Any employee scheduled to work on a holiday who reports <br />sick shall be charged sick leave for that holiday, provided the proper <br />NOMBL medical form has been completed. and shall forfeit his right to <br />~ receive the holiday pay. If the employee, has no sick leave or does <br />not have the proper PIOMBL medical form completed he shall recei~%e no pay <br />for that day. <br />ARTICLE XIX <br />LEAVES OF ABSEIVCE AI•!D F UIlERAL LEAVES <br />Section 19.01. All employees shall be entitled to take a personal <br />leave of absence with the approval of the Service Director, which <br />approval shall not be unreasonably withheld. Any leave of absence for <br />personal reasons, shall not, however, exceed a ninety (90) day period, <br />said employee in order to maintain any and all benefits which are paid by <br />the City shall be required to pay the Finance Director, the cost of <br />said benefits in advance on a monthly basis. The maximum permissible <br />leave of absence for any reason shall be ninety {90} days. <br />Section 19.02. Any employee who is required to be away from work due <br />to sickness shall be entitled to use up sick leave which has been accumulated, <br />according to the rates as specified in Sec. 18.05. ar~d Sec. 18.06. tlhen an <br />employee has exhassted all accumulated sick leave, thin that employee shall be <br />entitled to a sickness leave of absence which shall not exceed ninety (90) <br />days from the date that said employee has exhausted his accumulated <br />sick lea=~~e. During said ninety (90) day period, said employee, in order <br />to maintain any and all benefits which are paid by the City shall be <br />required to pay the Finance Director, the cost of said benefits in <br />adj;ance on a monthly basis. <br />13 <br />
