(a) Paid sick leave shall be earned and accumulated at <br />the rate of one and one fourth (1 1/4) days for <br />each month; <br />(b) Pay for sick leave shall be at the employee's <br />straight time hourly rate (or portion thereof if <br />absent for less than a full day); <br />(c) If an employee is absent on sick leave for more than <br />three (3) consecutive work days, the City shall require <br />the employee to present medical evidence that the <br />employee was under a doctor's care and is able to <br />return to work. The City will review the attendance <br />records of employees periodically and at least once <br />in each three (3) month period. If it believes that <br />an employee is developing an attendance problem, it <br />will notify the employee in writing that medical <br />evidence will be required for absences of three (3) <br />days or less. When an employee's record becomes <br />satisfactory, the requirement for such medical <br />evidence for short term absences will be removed, and <br />the employee will be notified in writing. <br />(d) Employees may use up to fifteen (15) work days per year <br />of earned and accumulated sick leave if necessitated by <br />illness in the employee's immediate family. For <br />purposes of this subsection, "immediate family" shall <br />be interpreted to be spouse; children; parents of the <br />employee, regardless of the place of residence; and <br />other relatives living within the household of the <br />employee. <br />(e) When an employee with five (5) or more years of service <br />retires, having received P.E.R.S. pension and having <br />accumulated sick leave time, the retiring employee may <br />convert one-half (1/2) of the accumulated sick leave <br />time to pay on the basis of his rate of pay at the time <br />of retirement, but the maximum amount paid shall not <br />exceed three hundred and sixty (360) paid hours. <br />(f) An employee should make an effort to schedule medical <br />and dental appointments for himself and his family <br />members during non-work hours. <br />(g) Sick leave taken the day prior to or the day after a <br />scheduled vacation or holiday will require a medical <br />excuse prior to approval for payment. <br />Section 4.
