(c) If an employee is absent on sick leave for more than <br />three f3) consecutive work days, the City shall require <br />the employee to present medical evidence that the <br />employee was under a doctor's care and is able to <br />return to work. The City will review the attendance <br />records of employees periodically and at least once <br />in each three (3) month period. If it believes that <br />an employee is developing an attendance problem, it <br />will notify the employee in writing that medical <br />evidence will be required for absences of three (3) <br />days or less. When an employee's record becomes <br />satisfactory, the requirement for such medical <br />evidence for short term absences will be removed, and <br />the employee will be notified in writing. <br />(d) Employees may use, when necessary, up to five (5) days <br />of earned and accumulated sick leave for the purpose <br />of attending the funeral of a member of their immediate <br />family. The term "immediate family" for purposes of <br />this subsection shall mean: parents; parents of <br />spouse; spouse; children of the employee; brother; <br />sister; brother of spouse; sister of spouse; grand- <br />parents; grandchild; as well as relatives living in <br />the employee's home. <br />(e) Employees may use up to fifteen (15) work days per <br />year of earned and accumulated sick leave if <br />necessitated by illness in the employee's immediate <br />family. For purposes of this subsection, "immediate <br />family" shall be interpreted to be spouse; children, <br />parents of the employee, regardless of the place of <br />residence; and other relatives living within the <br />household of the employee. <br />(f) When an employee with five (5) or more years of service <br />retires, having received P.E.R.S. pension and having <br />accumulated sick leave time, the retiring employee <br />may convert one-quarter (1/4) of the accumulated sick <br />leave time to pay on the basis of his rate of pay at <br />the time of retirement, but the maximum amount paid <br />shall not exceed two hundred and forty (240) hours <br />pay. <br />(g) An employee should make an effort to schedule medical <br />and dental appointments for himself and his family <br />members during non-work hours. <br />23 <br />
