_ Section 1: All leaves of absence when known in advance (and <br />any extensions thereof) must be applied for and granted in <br />writing on forms provided by the City (copy to the employee). An <br />employee and the IInion will be notified within three (3) working <br />days from the date the application was made of approval or denial <br />of the request leave. <br />Section 2: An employee shall accumulate seniority during <br />any leave of absence, but he shall not be entitled to any <br />benefits, except hospital/surgical benefits which shall be <br />provided by the City on a one hundred percent (1000 basis, <br />limited to a three (3) month period after an initial thirty (30) <br />day waiting period. An employee must direct pay any required <br />hospital/surgical premiums to the City on or before the monthly <br />due date. <br />Section 3: When an employee returns to work after a leave <br />of absence, he will be assigned to the position which he formerly <br />occupied at the then current rate of pay or to a similar position <br />if his former position no longer exists if he is physically <br />qualified for the job. In tha event the City questions whether <br />the employee is physically able to do the work, it will require <br />the employee to furnish a medical certificate of a physician <br />24 <br />-. <br />
