For purposes of this subsection, "immediate <br /> family" shall be interpreted to be spouse; children; <br /> parents of the employee, regardless of the place.sf <br /> Y <br />residence; and other relatives living within the'"` <br /> household of the employee. <br />(f) When an employee with five (5) or more years of <br /> service retires, having received P.E.R.S. pension <br /> and having accumulated sick leave time, the retiring <br /> employee may convert one-quarter (4) of the accumulated <br /> sick leave time to pay on the basis of his rate of pay <br /> at the time of retirement, but the maximum amount paid <br /> shall not exceed two hundred and forty (240) hours pay. <br />(g) An employee should make an effort to schedule medical <br /> and dental appointments for himself and his family <br /> members during non-work hours. <br />(h) Sick leave taken the day prior to or the day after a <br /> scheduled vacation or holiday will require a medical <br /> excuse prior to approval for payment. <br />35. General Leave Provisions. <br />(a) All leaves of absence when known in advance (and <br />(and any extensions thereof.) must be applied for and <br />.granted in writing on forms provided by the City <br />(copy to the employee). An employee and the Union <br />will be notified within three (3) working days from <br />the date the application was made of approval or <br />denial of the request leave. <br />-14- <br />
