living within the household of the employee. <br />(f) 6Jhen an employee with ten (10) or more years of <br />service retires having accumulated sick leave time, <br />the retiring employee may convert one-quarter (1/4) <br />of the accumulated sick leave time to pay on the <br />basis of his rate of pay at the time of retirement, <br />but the maximum amount paid shall not exceed two <br />hundred and forty (240) hours` pay. <br />37. General Leave Provisions: <br />(a) All leaves of absence when known in advance (and <br />any extensions thereof) must be applied for and <br />granted in writing on forms provided by the City <br />(copy to the employee). An employee will be notified <br />within three (3) working days from the date the <br />application was made. <br />(b) An employee shall accumulate seniority during any <br />leave of absence, but he shall not be entitled to any <br />benefits, such as insurance coverage, unless the em- <br />ployee pays the amount of the required insurance <br />premiums to the City on or before the monthly due date. <br />(c) When an employee returns to work after a leave of absence, <br />he will be assigned to the position which he formerly <br />occupied at the then current rate of pay or to a <br />-22- <br />
