ARTICLE 25 - HOSPITALIZATtON <br />Section 1. The City shail provide elther individual or family medical insurance coverage at the <br />employees option for each fuil-time empioyee. The employer shall have the option of seeking <br />aitemate health insurance providers or plans and wil( attempt ta offer at least two (2) different plans. <br />Basic hospitalization wiil be consistent with and similar to those plans offered on Jufy 1, 9994. Basic <br />hospifa(ization wiil be tha# offered to a!l other employees of the City of North Olmsted. The employer <br />has the right to nego#iate capayment and prescription drug rates with providers to lower the cost to <br />the empioyer as weil as the employee contribution. <br />Section 2. Effective Jufy 1, 1998, through December 31, 1999, fuA-time employee(s) participating <br />within either an individuai or family plan shall contribute an amouht equa) to ten (10%) percent of the <br />plan premium or expected cost of such medical plan and the er?ployer shall contribute ninety (90%) <br />? <br />percent oi such cost. The employee contribution shall be withheld via payroll deduction not (ater <br />than the first pay period each month. <br />Effective January 1, 2000, and through the duration of this agreement, full-time employee(s) <br />participating within either an individual or family plan shall contribute an amount equal to twelve <br />(12°/a) percent of the plan premium or expected cost of such medical plan and the employer shall <br />contr+bute eighty-eight (88°l0) percent of such cost. The empfoyee contribution sha(I be withheid via <br />payroll deduction not later than the first pay period each month. <br />Section 3. Expected cost or premium is defined as the cost of a!I medical, hospi#al, prescription <br />drug and related fees, including but not limited tv, administration fees. <br />Section 4. Tha employer reserves the right to establish all plans, enrollment periods and <br />regulations. <br />Section 5. The employer shaf( make an eamest effort to establish a committee consisting of one <br />(1) representative from each of the City bargaining units, if they choose to be represented and a <br />minimum of one (1) representative of the emp{oyer.
