with the terms of this Agreement and shall include disputes conceming disciplinary actions. W hen <br />any such grievances arise, the fo!lowing procedure shail be observed: <br />Ai( grievances shall be written and filed on an Official Grievance Form. The Official <br />Grievance Form shail be provided to the employees by the Union and shall contain the fol(owing <br />information at the time the gdevance is fiisd without exception: <br />• The date the gdevance occuRed; <br />• The time the gdevance occurred; <br />• Provide a detailed description of the incident giving rise to. the grievance; <br />• Cite specific Articles and Sectians of the Agreement involved and their direct relationship to <br />the incident; <br />• Relief Requested; <br />• Name and signature of the smployee and the date signed by the employee; <br />• Name and signature of the Steward and the date signed by the Steward; <br />Step 1: An employee who has a grievance shall reduce it to writing and the gdevance shall be <br />presented to the immediate supervisor by his steward, in the presence of the employee, within <br />seven (7) working days after the empioyee leamed or should have leamed of the event upon <br />which the gdevance is based. The gdevance shall be presented within the lastfifteen (15) minutes <br />of the empioyees respective shift. The supervisor shall meet with the empioyee or the empioyee <br />and his steward within seven (7) working days. The supervisor shall give an answer in writing to <br />the participants involved in the initial meeting within seven (7) working days following the date of <br />the grievance discussion. The af#ected employee shall be required to attend the aforementioned <br />meeting. <br />Step 2: If the employee's gdevance is not satisfactorily settled at Step 1, the gdevance shall be <br />submitted to the Department Director within seven (7) working days after the Step 1 answer. The <br />Director and/or his designee repr.esentative or representatives shall meet the Local Union <br />President, the steward or his designee within seven (7) working days afterthe gdevance has been <br />ems:opeiu/1794af1-cio -10- <br />_
