(b) Pay, for sick teave shail be at the employee's straight time haurfy rate (or portion thereof if absent for tess than a full day); (c) if an employee is absent on sick teave for more than three (3) consecutive work days, the City shall require the employee to present medical evidence that the empioyes vrras urtder a doctor's care and is abis ta retum to work. The City wii( review the attendance records of employees periodicaily, and a# least once in each three (3) month period. If it believes that an employee is developing an attendance problem, it wiit notify the employee in wribng that medical evidence will be required for absences of three .(3) days or less. When an employee's record becomes satisfac#ory, the requirement for such medical evidence for short term absences wi1l be removed, and the employee wiil be notified in writing. (d) Employees may use up to fifteen (15) work days per year af eamed and accumulated siek Ieave if necessitated by iilness in the empiayee's immediate family. For purposes of this subsection, "immediate family" shatl be interpreted to be spause; children; parents of the employee, regardle.ss of the place of residence; and o#her relatives living within the household of the empioyee. (e) When an employee with five (5) or more years of seivi.ce retires, having . received PERS pension, dies or takes clisability retirement and has accumulated sick Ieave time, the retiring employee may convert one-half ' (1/2) of the accumulated sick leave time (with the City of North Olmsted) to pay on the basis ofhis rate ofpay at tb.e time of retirement, but the maximum . am.ount paid shall not exceed nine hundred and sixty (960) hours. Employees hired after July 1, 1998, and have accumulated sick leave with another political subdivision wM transfez said sick leave and shall use the transferred sick leave prior to sick leave accumulated while employed with the City of North Olmsted. Sick Ieave carry over from another politioal subdivision shall not be used to calculate an employees buy-out pursuant to the above. (f) An employee shoutd make an effort to schedule medicai and dentai appointments #or himself and his family members during non-working hours. (g) Sick leave taken the day prior to or the day after a scheduled vacation or holiday wiil require a medicat excuse prior to approval for payment. Section 4. Parentat Leave: All emplayees shall be entitled to parental leave of absence upon the birth or adoptian of a chiid.
