<br />household of the employee. <br />(e) When an employee with five (5) or more years of service <br />retires having received P.E.R.S. pension, dies, or <br />takes disability retirement and has accumulated sick <br />leave time, the retiring employee may convert one-half <br />(1/2) of the accumulated sick leave time to pay on the <br />basis of his rate of pay at the time of retirement, but <br />the maximum amount paid shall not exceed the.following: <br />Five hundred (500) hours in period 7/19/92 - 6/30/93 <br />Six hundred (600) hours in period 7/1/93 - 6/30/94 <br />Seven hundred (700) hours in period after 7/1/94 <br />(f) An employee should make an effort to schedule medical <br />and dental appointments for himself and his family <br />members during non-work hours. <br />Section 2: Consistent with the just cause standard in <br />Article 30, the City may discipline or, if appropriate, discharge <br />an employee for abuse of sick leave or the patterned use of sick <br />leave (Ex. repeated Mondays, Fridays, or days after payday). <br />ARTICLE 16: GENERAL LEAVE PROVISIONS <br />Section 1: All leaves of absence when known in advance (and <br />any extensions thereof) must be applied for and granted in <br />writing on forms provided by the City (copy to the employee). An <br />employee and the Union will be notified within three (3) working <br />days from the date the application was made of approval or denial <br />of the request leave. <br />Section 2: An employee shall accumulate seniority during <br />any leave of absence, but he shall not be entitled to any <br />benefits, except that the City shall continue to pay its portion <br />of the employee's hospitalization insurance in Article 25 for a <br />three (3) month period.
