Establish an expiration time/date for the PRCS operation. <br />8. Complete and issue the Entry Permit, and maintain a copy for future <br />reference. <br />9. Conduct an annual review of the PRCS program, using past completed <br />Entry Permits. Maintain a written record of this. <br />C. The simplified entries into a PRCS noted in "C" above involve the following <br />steps (which require verification from the entry supervisor); <br />1. Completion of employee training. <br />2. Removal of entry cover safely, e.g. watch for pressure and explosive <br />potentiaL <br />3. Guard opening created by removal of the cover promptly. <br />4. Test space for oxygen content, LEL, and toxic air contaminants to PEL <br />levels. <br />5. Retest periodically for oxygen content, LEL, and toxic air contaminants. <br />6. When a hazard is detected: <br />a. Leave the conf'med space. <br />b. Reevaluate the site. <br />c. Record hazard and report to authorities. <br />7. Maintain written documentation of the entry (See Attachment C, <br />"Simplified Permit") and include: <br />a. Date <br />b. Location <br />c. Signature <br />E. If any entry will be made into a PRCS by a contractor employee, a visitor, <br />official, or any regulatory personnel, the entry supervisor must notify that person <br />5
