Bohlmann, Service Director <br />City of North Olmsted <br />Re: Request for Proposal Parking Lot and ADA <br />Improvements at Springvale Country Club <br />GSR Proposal No. 95-37 <br />Page 2 of 4 <br />? <br />? <br />island, perimeter curbing, pole lighting, landscaping, irrigation system along <br />Canterbury Road and painted lines for parking stalls. <br />5. The ground sign is based on aluminum letters mounted on a new brick wall with <br />ground set lighting of the letters. <br />6. The parking lot lighting will be on new poles with wiring connected to the <br />main electrical distribution center on the west basement wall of the Ballroom <br />Building. Photocells will be used for security lights and a time clock will <br />control all other light fixtures. The existing light pole at the north side <br />of the present parking lot will be removed. <br />7. The irrigation system along Canterbury Road will be fed from the existing golf <br />course system or a new service from the city water main. <br />Clarifications related to Building Improvements stated in your RFP letter are: <br />1. A ramp for the disabled and deliveries to the kitchen will be installed north <br />of the present Ballroom Entrance. The ramp will split at the west exterior <br />wall of the building and have ramps going north to enter the new Toilet Room <br />area and south to enter the kitchen door. The Entrance Foyer roof will be <br />modified to allow head room at a new entrance door to the Toilet Room area. <br />A roof over the ramp may be desirable but is not included in the base fee - <br />see paragraph on Supplementary Services. <br />2.' The present Office and Storage Rooms on the north side of the Entrance Foyer <br />will be remodeled to provide a Handicapped Entrance and Men's and Women's Toilet <br />Rooms. The Toilet Rooms will meet ADA requirements and contain one water <br />closet and one lavatory per room for use by the disabled. The temperature <br />control panel will be relocated. Plumbing piping will be connected to existing <br />piping in the basement. <br />Our proposed lump sum fee for these services is: <br />1. Survey per paragraph No. 2 of the RFP $ 1,200 <br />2. Design and construction document services per <br />paragraph Nos. 1 and 3 through 5 of the RFP <br />- Base Bid Work 13,100 <br />- Alternate for North Expansion of the Parking Lot 1.000 <br />Total Design Fee <br />$ 15,300 <br />If the City desires, our office will base our services on actual hours expended <br />plus direct costs. The maximum fee will not exceed the above fixed fees. Monthly <br />invoices for services performed will list employees by discipline, hourly billing <br />rate from the schedule below times the hours spent by each person on this project <br />during the billing period. <br />
