removal, or any similar charges by the city; <br /> (2) Any fees, fines, or penalties, of any sort arising from provisions of the <br /> property maintenance codes. <br /> (c) A license is valid for a period of one (1) year from the date of issuance unless <br /> suspended or revoked for cause by the city. Licensees must apply for license renewal sixty(60) <br /> days prior to the expiration date of the license. Sale of the subject property results in immediate <br /> termination of any license. Failure to obtain or timely renew a license is a violation of this <br /> Chapter. <br /> (d) License fees for rental units are established in Chapter 1321 Permit and Inspection <br /> Fees. A license fee, once tendered, may not be refunded or transferred. <br /> 1319.04 APPLICATION FOR LICENSE. <br /> Application forms will be available in the Division of Building and shall include at least <br /> the following information and any other information that may be requested: <br /> (a) The name, address and telephone number of the owner of the property; <br /> (b) If the owner is a limited partnership, corporation or limited liability company, the <br /> name, address and phone number of the statutory agent of such entity; <br /> (c) If the owner is a general partnership, the name address and phone number of the <br /> general partner to receive notices relating to the rental unit(s); <br /> (d) The address of the property and the number of rental units located at the property; <br /> (e) The name, address and telephone number of any agent or manager of the <br /> property; <br /> (f) A description of the rental units including the type and number of rooms as well <br /> as the square footage of each room; <br /> (g) The name of each person residing in each rental unit as of the date of application; <br /> (h) The signature and name of the person submitting the application; and <br /> (i) The date the application is executed. <br /> 1319.05 INSPECTIONS. <br /> The Building Commissioner or his agent, is hereby authorized to make, or cause to be <br /> made, inspections of all structures or premises containing rental units to determine whether such <br /> structures or premises comply with City Ordinances, the Fire Prevention Code, and State laws. <br /> The Commissioner shall notify the owner of any rental unit that is to be inspected, or his agent, <br /> that arrangements must be made with the Division of Building for an inspection appointment <br /> within ten(10) days of such notice. The owner or agent, upon consent, shall cause the entire <br /> structure to be accessible for inspection at the time arranged, which shall be during regular <br /> business hours. If the owner or agent, or an occupant, refuses to consent to an inspection of the <br /> subject property, or if consent is otherwise unobtainable, the Commissioner or his or her agent <br /> shall not make such inspection without first obtaining a search warrant. No license shall be <br /> issued or renewed if any violation of City Ordinances, the Fire Prevention Code or State law <br /> exists. <br /> 1319.06 SUSPENSION OF LICENSE. <br /> The Building Commissioner may suspend the license required in Section 1319.03, upon <br /> finding or being advised by the Fire Chief or Police Chief that such premises are in violation of <br /> any provision of this chapter or any other ordinance or resolution of the City. <br />
