Payr7rent shall be based upon the Basic Hourly Rate <br />of Pay tt-)r his "\,\,ork week" at the tirrrc of disahility, retirement or death plus Longevity and the <br />amu <br />ont shall be one-half'( 1112) of the unused Sick i.e. with a maximum payment of one <br />thousand ( 1.000) hours. <br />15A Any employee who transfers from are department within the City of North C)lmsted to <br />another shall be credited with the unused balance of Sick Leave accu1171.1lated. <br />15.5 Sick Leave may be taken for any of the 1'i311oiv ing reasons: <br />A. Illness Or injury to the ernplo�ee. <br />B. Illness or injury of a member ofthc employees immediate family. <br />C. Medical, dental, or optical examination or treatment of the employee. Nv here the <br />treatment may not be scheduled durim-, non -working hours. <br />D. Exposure to contagious disease where quarantined by the Board of I-lealth or <br />conununicable to other employees. <br />E. PregnancN of the employee:. birth cif a child, and placement of a child with the <br />employee for adoption or foster care. <br />15.6 The Employer shall require an emplo}ee to furnish a standard form identifying the reason <br />for the use of sick leave as per 15.5 and signed by the employee. <br />15.7 Before an absence may be charued against accumulated sick leave, the F-:mplayer may <br />require proof of illness. injury or death as may be satisfactory to the Employer. or may require <br />the employee to be examined by a physician designated by and paid for by the F;mployer. h7 any <br />event. an employee absent for three (3) consecutive work days must supply a physicians report <br />to be eligible for paid sick leave, unless waived by the Employer. This paragraph shall not be <br />applied in an arbitrary, capricious or unreasonable manner. <br />15.8 If the emplo}lee fails to submit adequate proof of illness, injury or death, or in the event <br />that upon proof as is submitted or upon the request of medical examination, the Employer finds <br />there is not satisfactory evidence of illness or death sufficient to justify the employee's absence. <br />such leave nnav be considered an unauthorized leave and shall be without pay. <br />15.9 Any abuse or patterned use ol`sicle leave shall be just and sufticierrt cause for disciplinary <br />action, including discharge. <br />
