The aforementioned pa}ment shall he made du►•in,L, <br />the first pa\ period of.lanuarv. each year ot'this Agreement for previous year service as a <br />designated training dispatcher. <br />ARTICLE 28: <br />UNIFORM MAINTENANCE ALLOWANCE <br />SECTION 1: All employees shall receive an annual uniform maintenance allowance in the <br />amount of six hundred seventy-five (567 .00) for contract years 2018, 2019 and 2020. 'I'll is <br />amount shall be paid in the reo.ular paycheck during the first pay period in December of each <br />year. <br />In addition, prohationary employees shall receive an advance of one-haIf`(112) of'the first <br />vear's uniform maintenance allowance upon appointment to the department. and one-ha11'( 1 /2) <br />after completion of six (6) months of service. <br />ARTICLE 29: <br />HOSPITALIZATION <br />SECTION I: The Employer shall provide, pursuant to Appendix A. single, family, ernploNee <br />and spouse, employee and child/children medical insurance coverage at the ernploy,ee`s option <br />for each full-time employee. The Employer shall have the option of'seeking alternate health <br />insurance providers or plans, providing coverage is equal to or better than provisions of <br />Appendix A. <br />SECTION 2: Employee(s) participating in the city's medical insurance coverage program shall <br />contribute an anununt equal to thirteen (13°,o) percent of the plan premium or expected costs of <br />such medical plan, and the Employer shall contribute eiollty-seven (87%) percent of such costs. <br />Me employee contribution shall be withheld via payroll deduction not later than the first pay <br />period each month. <br />All ernployee contributions, as required by this Article, shall be withheld via payroll <br />deduction not later than the first pay period each month for all the employee percentage <br />contributions stated above. <br />SECTION 3: For the year 2021. the Employer will offer HMO, PPO and HDI -JP -HSA plans <br />that provides for annual deductibles. co -pays and co-insurance similar to those as set torth in the <br />attached Plan designs at Exhibit A. providing said plans contain deductibles. co -pays, co- <br />insurance benefits that are nig less than those contained in Appendix A. Commencing in year <br />2022 and in 2023, the Employer vvill offer I IMO and I IDI IP -1 ISA plans that provide for annual <br />deductibles, co -pays and co-insurance similar to those as set forth in the attached Plan designs at <br />22 <br />
