1 1.02 Should an employee voluntarily resign during their probationarN period, and the City of <br />North Olmsted paid for their OPTA training. the employee is responsible to reimburse the citv <br />the frill training cost. The Employer iiia\ deduct said cost from the employees final settlement <br />check. <br />ARTICLE XII I IFAIAII INSURANCL <br />12.01 The I=.mployer shall provide. pursuant to Appendix A. single, Cam ilv, employec and <br />spouse, employee and child/children, medical insurance coverage at the employees option for <br />each full-time employee. <br />12.02 Employee(s) participating in the city's medical insurance coverage program shall <br />contribute an amount equal to 13°% percent of the plan premium or expected costs of such <br />medical plan, and the Employer shall contribute eight\ -seven (87�0) percent of such costs. The <br />employee contribution shall be withheld via payroll deduction not later than the first pay period <br />each month. <br />12.03 For the year 2021 only, the Employer \Nill offer FINIO, PPO and HDHP-HSA plans that <br />provide for annual deductibles, co -pays and co-insurance similar to those as set forth in the <br />attached Plan designs at Exhibit A, providing said plans contain deductibles, co -pays, and co- <br />insurance benefits that are no less than those contained in Appendix A. Commencing in \car <br />2022 and 2023, the Employer will offer I IMO and HDHP-1 ISA plans that provide for annual <br />deductibles, co -pays and co-insurance similar to those as set forth in the attached Plan designs at <br />Exhibit A (excluding the PPO option which is eliminated), providing said plans contain <br />deductibles, co -pays and co-insurance benefits that are no less than those contained in Appendix <br />A. <br />12.04 Employees shall participate in the AFSCME Dental Benefit flan and Vision Benefits of <br />America, Inc. (VBA) Vision Plan. <br />12.05 Expected cost or premium is defined as the cost of all medical. hospital. prescription <br />drug, and related fees including, but not limited to administrative fees. <br />12.06 The Employer reserves the right to establish all plans, enrollment periods and replations. <br />12.07 The Employer will secure a commercial insurance policy Nvhereby any bargaining unit <br />member killed in the line of duty shall receive to his spouse or estate, a payment ol' fifty <br />thousand ($50,000.00) dollars. Such payment will be made in accordance with the terms ol'the <br />commercial plan. <br />12.08 The Employer shall cause the Health Care Plan to be a qualified Plan under Section 125 <br />of the Internal Revenue Code. <br />
