employee contribution shall be withheld via payroll deduction not later than the Iirst pay period <br />each month. <br />12.03 For the year 2021 only the Employer will offer HMO. PPO and HDHP-HSA plans that <br />provides for annual deductibles, co -pays and co-insurance similar to those as set forth in the <br />attached Plan designs at Exhibit A. providing said plans contain deductibles, co -pays, and co- <br />insurance benefits that are no less than those contained in Appendix A. Commencing in vear <br />2022 and in 2023, the Employer will oiler I IMO and I IN IP -MSA plans that provide for annual <br />deductibles, co -pays and co-insurance similar to those as set forth in the attached Plan designs at <br />Exhibit A (excluding the PPO option which is eliminated). providin(, said plans contain <br />deductibles, co -pays and co-insurance benefits that are no less than those contained in Appendix <br />A. <br />12.04 Employees shall participate in the AISCME Dental Benefit Plan and Vision Benefits of <br />America, Inc. (VBA) Vision Plan. <br />12.05 Expected cost or premium is defined as the cost ol'all medical. hospital, prescription <br />drug, and related fees including, but not limited to administrative fees. <br />12.06 The Employer reserves the right to establish all plans. enrollment periods and regulations. <br />ARTICLE 13 CLOTHING ALLOWANCE <br />13.01 All full-time bargaining unit members shall receive an annual clothing allowance of <br />seven hundred twenty-five dollars ($725.00), pay able with the Iirst regular pay check in January <br />for the duration ofthis Agreement. (Clothing allowance shall be pro -rated per month in the <br />employee's first year of employment and final year ofemployment.) <br />13.02 In the event of damage to the uniform items, including, watches and prescription <br />eyeglasses, which damage occurs in the active discharge of an employee's duties enforcing the <br />law, the Employer shall pay the difference between the amount of reimbursement from insurance <br />or court ordered recovery, and the cost of repair or replacement at current fair market valire. <br />I owever, the Employer's monetary reimbursement obligation for repair or replacement of the <br />following articles shall not exceed the maximum amounts set forth below: <br />Item <br />Maxi111u171 Emplover Cost <br />Watch $ 30.00 <br />Prescription Eyeglasses $100.00 <br />Uniform .Jacket $200.00 <br />Line of duty damage shall be reported immediately to the Chief of'Police. On approval of' the <br />Chief of Police, the above -noted payment shall he forwarded to the employee, absent or after any <br />other form of reimbursement and upon receipt of replacemcrit of said items. <br />
