(a) Illness or injury to the employee <br />(b) Illness or injury of a member of the employee's immediate family as defined in Article <br />15.02 (d). <br />(c) Medical, dental, or optical examination or treatment of the employee, where the treatment <br />cannot be scheduled during non -working hours. <br />(d) Exposure to contagious disease where quarantined by the Board of Health or <br />communicable to other employees. <br />(e) Pregnancy of the employee, birth of a child, and placement of a child with the employee <br />for adoption or foster care. <br />An employee reporting off sick for the above reasons will be considered approved provisionally <br />subject to final review and approval by the Employer. <br />15.02. A full-time employee shall be granted sick leave for reasons outlined in Article 15.01 as <br />follows: <br />(a) Accumulation. Paid sick leave shall be earned and accumulated at the rate of four and <br />six -tenths (4.6) hours for every eighty (80) hours paid, excluding overtime; <br />(b) Rate of Pay. Pay for sick leave shall be at the employee's straight time hourly rate (or <br />portion thereof if absent for less than a full day); <br />(c) Documentation/Periodic Record Review. If an employee is absent on sick leave for <br />more than three (3) consecutive work days, the Employer shall require the employee to <br />present a physician's report attesting of the employee's incapacity and that the employee <br />was under the doctor's care and is able to term to work. The Employer will review the <br />attendance records of employees periodically and at least once in each three (3) month <br />period. If the Employer believes that an employee is developing an attendance problem, <br />the Employer will notify the employee in writing that medical evidence will be required <br />for absences of three (3) days or less. When an employee's record becomes satisfactory, <br />the requirement for such medical evidence for short term absences will be removed, and <br />the employee will be notified in writing. <br />(d) Immediate Family Use Limitations. Employees may use up to fifteen (15) work days <br />per year of earned and accumulated sick leave if necessitated by illness in the <br />employee's immediate family. For purposes of this subsection, "immediate family" shall <br />be interpreted to be spouse, children, parents of the employee, regardless of the place of <br />residence; and other relatives living within the household of the employee. <br />(e) Sick Leave Conversion/Severance.
