(a) Accumulation. Paid sick leave shall be earned and accumulated at the rate of four and <br />six-tenths (4.6) hours for every eighty (80) hours paid, excluding overtime. <br />(b) Rate ofPay. Pay for sick leave shall be at the employee's straight time hourly rate (or <br />portion thereof if absent for less than a full day). <br />(c) Documentation/Periodic Record Review. If an employee is absent on sick leave for <br />more than three (3) consecutive work days, the Employer shall require the employee <br />to present a physician's report attesting to the employee's incapacity and that the <br />employee was under the doctor's care and is able to return to work. The Employer will <br />review the attendance records of employees periodically, and at least once in each three <br />(3) month period. If it believes that an employee is developing an attendance problem, <br />it will notify the employee in writing that medical evidence will be required for <br />absences of three (3) days or less. When an employee's record becomes satisfactory, <br />the requirement for such medical evidence for short term absences will be removed, <br />and the employee will be notified in writing. <br />(d) Immediate Family Use Limitations. Employees may use up to fifteen (15) work days <br />per year of earned and accumulated sick leave if necessitated by illness in the <br />employee's immediate family. For purposes of this subsection, "immediate family" <br />shall be interpreted to be spouse, children, parents of the employee, regardless of the <br />place of residence; and other relatives living within the household of the employee. <br />(e) Sick Leave Conversion/Severance. When an employee with five (5) or more years of <br />service retires, having received PERS pension, dies or takes disability retirement and <br />has accumulated sick leave time, the retiring employee may convert one-half (1/2) of <br />the accumulated sick leave time (with the City of North Olmsted) to pay on the basis <br />of his/her rate of pay at the time of retirement, but the maximum amount paid shall not <br />exceed one thousand (1000) hours. <br />(f) Sick Leave Transfer/Order of Use. Employees hired after July 1, 1998, and have <br />accumulated sick leave with another political subdivision will be allowed to transfer <br />said sick leave and shall '- - iiwisfiiff- d - -' '-- - pli--_ �. .._.. <br />_:_I.. A ,.........a...ea <br />but such sick leave carry over from <br />another political subdivision shall not be used to calculate an employee's buy-out <br />pursuant to the above. <br />11.03 Designation of FML Oualifvine Sick Leave. If an employee is absent on sick leave or <br />other related reasons for more than three (3) consecutive work days and said absence qualifies <br />under the FMLA, the Employer may apply those days against the twelve (12) week leave period <br />11.04 Fitness for Duty Examinations/Disability Separation.
