ARTICLE 29 28: <br />HOSPITAL IZAT ION/ANCILLARY INSURANCE <br />SECTION 1. Medical Coverare. The Employer shall provide, , single, <br />family, employee and spouse, employee, and child/children medical insurance coverage at the <br />employee's option for each full-time employee. The Employer reserves the right <br />to establish all plans, enrollment periods and regulations. <br />. The Employer shall determine the costs, terms and conditions, and <br />benefit levels of all plan offerings and any alternative programs, which may be subject to <br />change, provided that such changes are similar from year to year. <br />SECTION 2. Employee Contributions/Premium Cost Components. Employee(s) participating <br />in the city's medical insurance coverage program shall contribute an amount equal to thirteen <br />percent (13%) of the plan premium or expected costs of such medical plan, and the Employer shall <br />contribute eighty-seven (87%) percent of such costs. All employee contributions, as required by <br />this Article, shall be withheld via payroll deduction not later than the first pay period each month <br />for all the employee percentage contributions stated above. ST6T4Q -& Expected cost or <br />premium is defined as the cost of all medical, hospital, prescription drug, and related fees <br />including, but not limited to administrative fees. <br />.o . As part of its plan offerings, the City may include HSA or HRA Plan Designs, which include <br />a City -provided HSA or HRA funding level, the level of which will be established by the City. <br />The parties agree that for the years 2014-2026, the HSAfunding level will be not less than fifty <br />percent (SOY.) of the annual deductible of individual and family coverage. The parties agree <br />that prior to establishing a HRA funding structure, it will meet and confer with the Union over <br />the effects of such action. <br />SECTION 4. Dental/Vision Coverage. Employees shall participate in the AFSCME Dental <br />Benefit Plan and Vision Benefits of America, Inc. (VBA) Vision Plan at current benefit levels. <br />Should availability change, the parties will meet for the purposes of identifying and selecting <br />comparable, uniform replacement provider(s). <br />SECTION 5. Life Insurance. All bargaining unit employees shall receive a twenty -five - <br />thousand -dollar ($25.000) in-group term life insurance policy in their time while employed by <br />the Employer paid for by the employer.
