Should an <br />employee voluntarily resign during their probationary period, and the City of North Olmsted paid <br />for their OPTA training, the employee is responsible to reimburse the City the full training cost. <br />The Employer may deduct said cost from the employee's final settlement check. <br />C ! Medical Coverage. The Employer shall provide, pursuant to Appendix A (as <br />updated from year to year to reflect plan offerings), single, family, employee and spouse, <br />employee and child/children, medical insurance coverage at the employee's option for each full- <br />time employee. 4386 The Employer reserves the right to establish all plans, enrollment periods <br />and regulations. (Note: Moved from below) The Employer shall determine the costs, terms and <br />conditions, and benefit levels of all plan offerings and any alternative programs, which may be <br />subject to change, provided that such changes are similar from year to year. <br />14.02. Employee Contribudons/Premium Cost Components. Employee(s) participating in <br />the city's medical insurance coverage program shall contribute an amount equal to thirteen percent <br />(13%) of the plan premium or expected costs of such medical plan, and the Employer shall <br />contribute eighty-seven percent (87%) of such costs. The employee contribution shall be withheld <br />via payroll deduction not later than the fust pay period each month. X3-45 Expected cost or <br />premium is defined as the cost of all medical, hospital, prescription drug, and related fees <br />including, but not limited to administrative fees. <br />As part of its plan offerings, the City may include LE4S or HSA Plan Designs, <br />which include a Cityprovided HSA or HSAfunding level, the level of which will be established <br />by the City. The parties agree that for the years 2024-2026, the HSA funding level will be not <br />less than fifty percent (50%) of the annual deductible of individual and family coverage. The <br />parties agree that prior to establishing a HRA funding structure, it will meet and confer with <br />the Union over the effects of such action. <br />4194 14.04. DentaWision Coverage. Employees shall participate in the AFSCME Dental <br />Benefit Planand Vision Benefits of America, Inc. (VBA) Vision Plan as long as such is available. <br />12 <br />4663-5]139656,-.5 <br />
